Changing Beneficiary

OTRS - The employee completes a new OTRS Form 1A application marking the beneficiary box.

ING - The employee may call ING to request a form to change a beneficiary.

Fidelity - The employee may call Fidelity to request a form to change a beneficiary. If removing a spouse, the spouse must also sign the appropriate form.

TIAA-CREF - The employee completes a new Beneficiary Designation form.

Vanguard - The employee may call Vanguard to request a form to change a beneficiary. If removing a spouse, the spouse must sign the appropriate form.
