Step 4: Complete Employee's
Withholding Allowance Certificate (W-4) |
The W-4 tells Payroll how much federal income tax to withhold from your paycheck. Employees without a W-4 on file will be taxed at the highest rate.
The federal withholding is applied to Oklahoma state taxes for HSC campus and program employees; OU employees may opt to complete a State Tax Withholding form (See #3). |
| 1. Complete the W-4 online. The form is found at the bottom of page 1 |
Open W-4
- Refer to instructions to complete the form.
- You must complete sections 1, 2, 3, 4, and 5.
- Section 1: provide the full address for mailing your W-2 at the end of the year.
- Section 1: enter your name and social security number AS THEY APPEAR ON YOUR SOCIAL SECURITY CARD.
- If you have lost your Social Security card or have changed your name, go to the Social Security website for instructions on getting a new card.
Go to the Social Security Administration website.
OPTIONAL FOR OUHSC EMPLOYEES IN PPP
OUHSC Professional Practice Plan (PPP) members (faculty only) can submit a separate W-4 form to be applied to the PPP Supplemental Payroll. The appropriate box at the bottom of the modified W-4 form must be checked to ensure the data will be applied to the correct payroll. Open modified W-4 form |
2. Print, sign and date the W-4 form. |
OPTIONAL FOR NORMAN CAMPUS AND OU PROGRAMS EMPLOYEES
Norman campus and OU program employees may choose to complete the State Tax Withholding form to ensure the appropriate state taxes are withheld or if you work out-of-state and wish to be exempted from paying Oklahoma state taxes. If the state form is not returned, your federal W-4 withholding selection will be applied to the state tax withholding.
